Your 990 Form or 990 EZ Form MUST include a Schedule A.

Local Officer Submission Form (.doc) You may submit this form three ways. Download it to your computer, type in your responses, re-save it and e-mail it as an attachment to membership@txpta.org. You may also print out the hard-copy version of the form to fill out manually and fax or mail to the state office.
